This form is to be used if you have been formally offered a Leadership Team position (board member or committee leader). You should carefully read and understand all of the Organizational Documents before acceptance. It is very important to us that you understand our mission, our beliefs, and how we operate.

Job Title

This is the Leadership Team position (board member or committee leader) that you've agreed to hold. Be sure you understand the description, requirements, and expectations before accepting. You are agreeing to hold this position for 1 year, and may be eligible to renew annually. (If, for some reason, you choose to step down at any time, or are unable to fulfill your duties - even temporarily - you should notify the ED or BC immediately.)

I have read and agree to all the organizational documents for The Alliance for Creative Outreach. I understand that these documents may be modified by the TACO Board through official processes, that changes will be published and communicated to all TACO members, and that my continued involvement indicates my agreement with such changes.
